Membership Refund Policy:

PASR membership payments are non-refundable.

 

Dental and Vision Insurance Cancellation/Refund Policy:

Any person who knowingly and with intent to defraud any insurance company or other person who files an application for insurance or statement of claim containing any materially false information or conceals for the purposes of misleading information concerning any fact material thereto commits a fraudulent insurance act, which is a crime and subjects such person to criminal and civil penalties. 

Premium payment is for 12 months of coverage and is not refundable for any reason. If a member does not renew the contract at the end of the 12 months, the member cannot re-enroll for 36 months. If a check or payment is returned due to insufficient funds, a $20.00 fee will be charged. Either you or your spouse must be a PAID Member of PASR to enroll or renew your dental and/or vision coverage. Premiums are not refundable if cancelled due to non-payment of dues.

If a policy is cancelled, a refund for the remaining months of the contract will be returned to the policy holder. The remaining months will be calculated to include the remaining months of the contract starting on the first day of the month subsequent to PASR receiving written notification that the policy is being cancelled. For instance, if PASR receives written notification on May 10th  that the policy holder wishes to cancel their policy whose contract year ends on July 31st, PASR will issue a refund in the amount equal to the monthly cost of the annual premium for the months of June and July. Cancellation of a policy will result in the member not being able to re-enroll for a period of 36 months.
