Step #1 -- There is a printable membership application form on the next page. If you wish to apply for membership, please print it out

Step #2 -- Complete the information requested on the form

Step #3 -- Make out a check payable to PASR for the amount indicated for the level of membership you desire

Step #4 -- Mail the completed form and check to PASR State Headquarters.
